I have been a registered nurse for two years this July! I began and continue to work as a telemetry nurse in the acute care setting. I am aiming to complete my BSN to open more doors for me within my career. I have great interest in becoming a critical care RN, and from there hopefully a NICU RN. Just as the title of this course states, I hope to come out of this course with a better understanding of the ethical and legal piece of nursing. Aside from schoolwork, I can be found spending my time with my boyfriend riding our bikes at the Cape Cod canal, or spending time building puzzles together. Now with the weather getting better, I can be found spending free time at the beach or spending time with my family at their pool. Recently, my telemetry floor at the hospital I work for was a strictly COVID unit, meaning I only cared for patients with COVID for two months. During this time, I discovered an ethical dilemma. The acuity of patients with COVID was so high. A lot of patients did not survive the virus, in fact I have never lost as many patients as I did from COVID in my entire career of 2 years. But thats not the saddest part, or the biggest dilemma, these patients were dying alone. Hospitals had restricted all visitors for the safety of themselves, the patients and the staff. This meant that during the last hours of some peoples’ lives, they were alone, and scared. “More than 5,000 people in Massachusetts have died from complications related to COVID-19, and many were alone and isolated when they died” (Schairer, 2020). Patients who were diagnosed with COVID got placed behind a closed door, completely secluded from their loved ones, and only got human interaction briefly when a nurse or doctor walked in. We were told to limit our exposure to infected patients, so contact with the patients during this time was very limited, leaving them scared and lonely. Because we had to continue to keep ourselves healthy to remain able to care for those who were COVID+, the patients suffered. They suffered physically as well as mentally and emotionally. Working as a nurse through this pandemic really stood out to me. I knew that patients were alone, and scared. I knew that patients were dying alone, behind a closed door. I began to spend more time with patients. I had patients who obviously scared and would look to hold my hand, so I would stay a couple minutes longer to let them do so. “COVID-19 steals the precious final days, hours, and moments spent together when a loved one is dying, leaving those grieving in emotional limbo, and sometimes feeling guilty that their loved one died alone” (Schairer, 2020). Family members were also scared and alone. I put myself in their position and can’t imagine the pain they felt losing a loved one to COVID. We typically bring bereavement carts to families filled with refreshments while they stay by their loved ones side during the last of their lives. COVID did not allow for this, causing ethical dilemmas within nursing, patients and family members. I love spending time with my two children do fun things like water parks or fairs in our free time. Right now, due to COVID we must settle for backyard pool and trampoline fun. My career goals are to finish my bachelor’s in medical administration and get a position at a hospital billing department or medical practice using my degree. I hope to gain insights into ethical and unethical practices and why ethically we need to bill properly to avoid audits and fines and why how we act towards others matters. I requested my own medical records when I moved from Texas and my doctor sent them but when I opened the envelope my records were not inside it was records for someone else with my name and address on the outside. This is a major error not only for HIPPA but also ethically because confidentiality of medical issues is so very important. I know for me I would have been so upset if I was to find out my medical paperwork went to someone else. I called the doctor to advise her that my paperwork wasn’t in the envelope it was someone else’s and all they said was ok sorry for the error you can shred the papers and we will get yours right out to you. They didn’t even ask me whose paperwork it was which I found this so unethical they should have wanted to call the patient and explain their mistake and clear up the mess they made. I don’t feel that the patient was treated fairly at all because of the lack of responsibility and consideration taken. Because they didn’t act like they were going to do anything, I called the person on the medical records myself and told her I had her records because of the mistake. She was so happy I had let her know and stated she was going to file a formal complaint against the doctor for her error due to her lack of communication and ownership of the problem that had occurred. In the future I think the provider needs to put better practices in place for HIPPA mailing and record filing and if even if an error still occurs after procedures are put in place to prevent this from happening then at least take responsibility for the error. This situation has made me realize even if a doctor is really good the ethical practices of the providers whole office needs to be ethical and run good practices in order for the providers over all service to be considered good.
